I feel like the ol' silent friend request thing happens a bit more frequently on here than certain other sites, and perhaps that is in part because the feature is not really labelled in a way that makes it clear what it does. "Befriend" doesn't quite hammer home the fact that the action needs to be reciprocal, and I think "Send Friend Request" would be better.

This sounds reasonable to me.
There are at least a couple of users whom I'm aware have engaged in serial befriending of characters they'd never even had contact with.

This would be good, though there will be improvements down the line towards both profiles and the inbox that should help even more with this issue. Fixing the "silent friend requests" issue is definitely on the docket, but how it'll get there will be a bit wait and see for now, but this is a simple and easy thing that can be implemented quickly.
